UCLA Long-Term Services and Supports (LTSS) Study Information Sheet
The purpose of this study is to understand the experiences of Californians who have difficulties with certain activities of daily life. We want to learn more about the types of services and supports people are aware of, what services or supports they are using, and how well their individual needs for assistance are being met.
We will ask you to answer a 15-minute survey. The survey will ask you about your individual needs for assistance, the amount and type of help you may currently receive, and the extent to which the help you get meets your needs. Your answers will be combined with the answers of other participants and shared with researchers to better understand the health and long term service and support needs of Californians.
Participation will involve a one-time telephone or online survey of about 15 minutes.
After completing the survey, you will be provided with a $25 gift card.
Some survey questions may be about things that are difficult for you to talk about. You will NOT have to answer any questions that you do not want to answer and you can end the survey at any time.
There will be no direct benefit. The study findings may be used to improve access to services and supports that may help improve the quality of life for Californians like you.
Yes, any information that is obtained in connection with this study will remain confidential. We will not collect any personally identifying information, such as your name, that could result in your identification.
You can choose whether or not you want to be in this study, and you may withdraw your consent and discontinue participation at any time. Whatever decision you make, there will be no penalty to you, and no loss of benefits to which you were otherwise entitled. You may refuse to answer any questions that you do not want to answer and still remain in the study.
